Sacramental Record Requests

If you received your sacraments at Sacred Heart or St. John Nam Catholic Church (3663 White Plains Rd, Bronx, NY 10467) and need a copy of your sacramental records, please fill out the following form. We will contact you as soon as it’s ready.

Please note that if you are requesting the records for an upcoming marriage, the certificate must be dated within 6 months of the wedding date.
